the #inserted method for modules

Would be cool to have an equivalent of the Ruby self.inserted method for modules, so that when a module inserted into a class we could do some additional mambo jumbo inside them.

I've added both included and extended versions support with names like
'selfIncluded', 'self_included' 'selfExtended', 'self_extended'
